A disease management program (DMP) is an organizational approach to medical care in which the treatment and care processes of patients are coordinated over the entire course of a – chronic – disease and across the boundaries of individual service providers and optimized on the basis of medical evidence. Since the introduction of the first DMP Diabetes Type 2 in 2003, all programs (indications: Diabetes mellitus type 1 and 2, breast cancer, CHD, bronichal asthma and COPD) of all health insurance funds have been comprehensively evaluated on the basis of §137f of the German Social Code Book V and the DMP Requirements Guideline. In a consortium consisting of infas and Prognos, infas as consortium leader conducts the evaluation on behalf of the Allgemeine Ortskrankenkassen and the Knappschaft. Für das DMP werden Patientenmerkmale und medizinische Dokumentationsdaten auf Individualebene verknüpft und mittels Quer- und Längsschnittanalysen sowie Ereigniszeitanalysen mit bis zu 33 Messzeitpunkten (entsprechend 16,5 Jahren) systematisch ausgewertet. Bis 2013 wurden zudem Erhebungen und Auswertungen zur Lebenszufriedenheit vorgenommen; dazu wurden pro Jahr bis zu 120.000 schriftliche SF36-Fragebögen versendet. Zu den Datenarbeiten gehören insbesondere:

Annahme der Prozessdaten aus verschiedenen Quellen (Kassen, Datenstellen)
Prüfung der Daten (Vollständigkeit, Plausibilität)
Nachforderung von fehlerhaften von Datenlieferungen
Datenimporte
Datenaufbereitung nach festgelegten Kriterien
Ziehung von Zufallsstichproben und schriftliche Patientenbefragung (bis 2013)
Verknüpfung der Daten aus verschiedenen Datenquellen auf Individualebene
Datenauswertung insgesamt und nach Subgruppen
Erstellung der Evaluationsberichte
