A disease management program (DMP) is an organizational approach to medical care in which the treatment and care processes of patients are coordinated over the entire course of a – chronic – disease and across the boundaries of individual service providers and optimized on the basis of medical evidence. Since the introduction of the first DMP Diabetes Type 2 in 2003, all programs (indications: Diabetes mellitus type 1 and 2, breast cancer, CHD, bronichal asthma and COPD) of all health insurance funds have been comprehensively evaluated on the basis of §137f of the German Social Code Book V and the DMP Requirements Guideline. In a consortium consisting of infas and Prognos, infas as consortium leader conducts the evaluation on behalf of the Allgemeine Ortskrankenkassen and the Knappschaft.
